At this time, COVID-19 evaluation and testing is only available to patients with symptoms. No appointments necessary. If you do not have COVID-19 symptoms, please contact your local pharmacy to schedule a COVID-19 test. We apologize for this inconvenience and hope to offer this service again soon.

You will need to make a telemedicine appointment to meet with the AFC Urgent Care provider to evaluate you for this test. During that video visit, you will be given instructions on how to receive this test at the urgent care center.
At this time, there no cost to insured or uninsured patients for either the PCR test or the telemedicine appointment.

For a telemedicine visit exclusively for COVID assessment, PM Pediatrics will not require a copay at the time of service. A claim will be submitted to your insurance to determine your individual financial responsibility. View PM Pediatrics’ participating insurance companies. Patients with Non-Participating Commercial Insurance or Medicare will be billed a total charge of $65. If the telemedicine provider determines that a Rapid Antigen test is indicated, there will be an additional $35 billed. For those not covered by insurance, PM Pediatrics will submit a claim to the appropriate state program. If you do not qualify for the state program, you will be billed as above.
For in-office COVID assessments, regular copays apply (patients with NY Empire plan will have an additional copay if a Rapid Antigen test is performed). If you are not covered by insurance or have Non-Participating Commercial Insurance or Medicare, you will be charged $65, if a PCR or antibody test are performed or $100 if a rapid antigen test is performed at the time of the visit.
The COVID PCR and Antibody tests are special tests, which will be sent to an outside laboratory. The laboratory will submit a claim to your insurance and will determine if you have any additional financial responsibility. Please contact your insurance to determine your financial responsibility for the laboratory fees. If you are not covered by insurance, the laboratory may bill you for the full cost of the tests, unless you live in a state that has waived fees for uninsured patients.

Every individual who wants a test must make an appointment at MinuteClinic.com. To receive a COVID-19 PCR test, you will be required to fill out the online survey to see if you qualify for a test, and then you will be able to make an appointment. Because of limited supplies and in accordance with CDC guidelines, testing is limited to certain patients.
