Finding a Doctor
Expenses at doctors not listed under Devon Health System (out-of-network) are payable at 60% (instead of 80% for in-network) billable to the patient.
ReferralsA referral is not required to see a doctor or specialist, but the deductible will be lower if seen first at Health Services. When Health Services is closed, there is no need to be seen there first.
Pharmacies Go to www.medco.com and register using Group Number and Member Number, found on insurance card, to access a complete list of area pharmacies.
Prescriptions Patient co-pay is $10 for generic or $30 for brand-name drugs.
To fill prescriptions before receiving an ID card, fill the prescription and pay in full for it, saving the receipt. When you receive your insurance ID card, it will include instructions on how to file a claim for reimbursement for prescriptions filled before you receive your card. The reimbursement form is available at www.cirstudenthealth.com.
Usual, Customary, and Reasonable Expense (UCR)
Deductible A deductible is the amount a patient pays before the insurance company begins to pay.
When Health Services is closed, there is no need to be seen there first.
Insurance Claim Forms are available online at www.cirstudenthealth.com
