Health Forms

All full-time undergraduate students are required to complete a physical examination along with an up-to-date immunization record.

Please make two additional copies of all forms: one for your records at home and one for participation in intramural or club sports activities.

To help make the transition to college run smoothly, please read the following Dos & Don’ts prior to filling out your health forms.

Fill out both pages of the required Health History form.
Schedule your immunizations in a timely manner to meet Cabrini’s deadlines:
July 15 for the fall semester and January 15 for the spring semester.
Have your health-care provider fill out the Physical Examination and Immunization forms.
Submit a copy of your insurance card, front and back.
Double-check the contents of all forms to make sure they are complete, especially the signatures and dates.
Make copies of all forms before sending to Health Services.
Health Services will only keep your medical records for seven years.
Specify “Health Services” on your return envelope if mailing forms.
If you are faxing, include a cover sheet with a contact phone number, and be sure the student’s name appears on each sheet.
Check Cabrini Vision to verify receipt of medical records.
Set up your Cabrini email account, which will be used verify receipt of insurance information.

DON’T

Don’t turn in your medical records to anyone other than the Health Services staff.
Don’t mail Athletics medical forms to Health Services or vice versa.
These are separate departments that cannot share confidential information.
Don’t mail your health records and then fax them, too. Just do one or the other.
Don’t call Health Services to verify receipt of your medical information. You will be able to check receipt on Cabrini Vision.
Don’t submit incomplete forms. Incomplete forms will impact your ability to start class!
If there is something missing from your forms, our staff will notify you in writing by mail.
