Health Forms

All full-time undergraduate students are required to complete a physical examination along with an up-to-date immunization record. Please print a copy of all forms.

The student fills out the personal health history form, pages 1 and 2. The student’s physician will complete the physical examination and immunization record form.

All forms should then be faxed (if possible) to Health Services at 610-902-8282 or mailed to Cabrini College Health Services, 610 King of Prussia Road, Radnor, PA 19087. Please retain copies of the forms for your records.
