To request review, please complete the Request for Protocol Review form and submit it via email as an attachment—with all supporting documents requested—to IRB@cabrini.edu or print out, complete, and deliver to the Faculty Support Office, Grace Hall Room 203.
The Institutional Review Board will initiate the review of protocol within seven business days and contact you about the status of your review and provide your IRB protocol number.
During winter and summer breaks, the review period will be extended.
If protocol requires an expedited or full review—where more than one IRB member reviews—the timeline for approval could be elongated (see Types of Review).
If revisions are necessary, it could also extend the approval timeline, depending on how quickly revisions are made.
Please check in directly with the assigned IRB member for protocol review status or revisions, if not yet approved.
You will receive an email confirmation when your protocol is officially approved. At that time, you may begin conducting the research.
Sample Consent FormsAll requests must also include samples of the consent forms that will be used.
A standard form is available for your use here, although you are free to adopt a different version.
General Consent form must include, if applicable:
*If participant is a minor (under 18 years of age), a special consent form must be used in addition to parental consent.
Protocol ContinuationIRB Protocol is valid for one year from date of approval. If you wish to extend your research past the one-year expiration, please submit a Project-Continuation request.
Project AmendmentIf any methods, materials, or samples change, please submit a Project Amendment.
Amendments do not extend the one-year expiration.
