To submit a request for review, please complete the Request for Protocol Review form. Complete the form online and submit via email as an attachment to IRB@cabrini.edu or print out and send to the IRB mailbox in the Faculty Support Office, Grace Hall, Room 203.
Sample Consent Forms
All requests must also include samples of the consent forms that will be used.
General Consent form must include, if applicable:
- Title of study
- General purpose of research
- What procedures participants will be involved in
- Any potential risks
- Any potential compensation
- Maintenance of confidentiality
- Freedom of withdrawal
- Contact information for primary investigator, in case participants have questions
