If you have a student who has the accommodation of extended time and you are able to provide it in your classroom or your office, please feel free to offer that to the student. Each student who has extended test time on his or her VISA (Verified Individualized Services and Accommodations) has been reminded that space is limited and it is his or her responsibility to request a test time in the testing room. - Please note that due to space and time constraints, occasionally a student may need to take an exam at a different time than given in class.
In order to ensure exam integrity, we have developed an accommodation Form. Please complete this form with every exam. The TAF has the following components: - The date that the test will be given
- The time allowed for exam in class
Generally, extended time for exams is 1.5 to 2 times the regular length of the exam. - Course number
- Student name
- Faculty name
- Notes for proctor
Any special instructions such as "open book allowed" or "use of one page of handwritten notes" can be given here. - Arrival time limitation
This should be the same as for students taking exam in class. - Pick-up/return of test
In order to make sure that you receive exams back in a timely manner, please indicate how you would like to pick up the exam.

Faculty Disagreements Regarding Appropriate Academic Adjustments / Accommodations If a faculty member has a concern about an academic adjustment recommended as appropriate for a particular student, the instructor should contact the coordinator of the Disability Resource Center immediately. If the instructor disagrees with the recommended accommodation, and in conjunction with the coordinator cannot identify an acceptable alternative that provides the student with equal opportunity to participate in the class, the instructor and coordinator should consult with the chair of the department as soon as possible to resolve the disagreement. If this effort is not successful, within five days the instructor should contact the dean of academic affairs who will review the matter and issue a final decision. The instructor is expected to provide the recommended academic adjustment until the matter is resolved. |
